The GPU Memory clock while posting this, on my Nvidia powered system, remains low as there is no significant load on the GPU. In the above screenshot the GPU Memory frequency is very low as there is not a significant load on the GPU.

Later I found out that Discord uses hardware acceleration. Turning it off solved my problem. To turn it off: In Discord, go to: Options > Appearance > Uncheck "Hardware Acceleration".
